Please arrive 15 minutes early for check-in and to complete intake forms.
If you arrive more than 15 minutes late, your appointment may need to be rescheduled and a $100 late fee may apply.
If you arrive less than 15 minutes late, we will do our best to provide your full service; however, your treatment time may be shortened to avoid delays for other clients. Full pricing still applies.
We kindly ask for at least 24 hours’ notice to cancel or reschedule your appointment.
Late cancellations or missed appointments will result in a $100 fee.
In the case of a medical emergency, documentation may be provided and the fee may be waived at the clinic’s discretion.
Prices, services, and products may change periodically as we continue to invest in the latest technologies and treatments.
Discounts and promotions cannot be combined. The highest value discount will be applied.
Beleza Clinic does not bill insurance, as our services are elective aesthetic and wellness treatments.

Packages are discounted for members only and must be paid in full at the time of purchase.
Unused treatments are non-refundable.
Packages are non-transferable.
All services must be completed within 365 days of the purchase date. Unused services will expire and are forfeited after one year.
The Pathway Membership Program includes three tier options and requires a 6-month minimum commitment.
Memberships auto-renew monthly unless cancelled in accordance with membership terms.
Unused credits do not roll over and must be used within the current billing cycle.
Banked credits are non-transferable and may only be used by the member to whom they were issued.
Gift cards may be redeemed for services only and cannot be used for products or redeemed for cash.
No refunds are provided once a service has been rendered.
Product returns within 7 days may be eligible for in-store credit only in the event of an allergic reaction.
Defective products may be exchanged within 7 days of purchase.
Prescription products are final sale and are not eligible for refund or exchange.
Results from aesthetic and wellness treatments vary from person to person and cannot be guaranteed.
While our providers use advanced techniques and technologies to achieve the best possible outcomes, individual results depend on many factors, including age, lifestyle, medical history, and adherence to recommended treatment plans.
By receiving treatment at Beleza Clinic, you acknowledge that no specific results have been promised or guaranteed.
Photos and/or videos may be taken for clinical documentation and treatment planning purposes.
If we would like to share images on social media or marketing materials, we will always obtain your written consent first. Personal identifiers will be blurred or removed.
By purchasing services, packages, or memberships from Beleza Clinic, you agree not to initiate a credit card chargeback or payment dispute for services that have been rendered or agreed upon.
If a payment dispute or chargeback is initiated, Beleza Clinic reserves the right to:
Suspend or terminate services and memberships
Charge the full balance of any discounted packages
Recover any costs associated with the dispute, including administrative and legal fees
Packages and memberships are intended for the sole use of the purchasing client or member and may not be shared, transferred, or split between individuals.
Beleza Clinic reserves the right to terminate or revoke package or membership benefits without refund if misuse, fraud, or policy violations are suspected.
Beleza Clinic reserves the right to refuse service to anyone for reasons including, but not limited to:
Medical contraindications
Safety concerns
Intoxication
Inappropriate or disruptive behavior
To help maintain a peaceful and relaxing environment:

While we do our best to accommodate provider requests, we cannot guarantee provider availability due to scheduling changes or unforeseen circumstances.
We value respectful interactions. Harassment, inappropriate behavior, or disrespect toward staff may result in termination of services without refund.
I agree to indemnify, defend, protect, and hold harmless the medical providers employed by Beleza Clinic and their respective officers, directors, employees, stockholders, assigns, successors, and affiliates (“Indemnified Parties”) from and against any and all liabilities, losses, claims, damages, judgments, settlement payments, deficiencies, penalties, fines, interest, and costs or expenses (including reasonable attorney fees) suffered, sustained, incurred, or paid by the Indemnified Parties in connection with, resulting from, or arising out of, directly or indirectly:
Medical care, services, advice, and/or treatment rendered by Beleza Clinic providers and staff
My failure to disclose all relevant information regarding my medical history, physical condition, medications, or allergies
Any acts or omissions related to medical care or pharmaceuticals provided directly or indirectly by Beleza Clinic providers and staff
I understand and acknowledge the potential side effects associated with BHRT pellets and hormone replacement therapy provided by Beleza Clinic.
I accept all risks associated with IV infusion therapy and injectable therapies and agree that I will not seek indemnification or damages from the Indemnified Parties.
We believe in transparency and informed care. We will always explain your treatments, benefits, and potential risks so you can make confident decisions.
